In order to identify the feedback from our patients i.e. those who have collected their disks, please enter the serial number of the disk that you collected (it is on the inner rim of the disk). Although we wont be able to identify you personally, it does mean that we know that the person filling in this on-line form is legitimate as we keep a list of the serial numbers of the batches of disks that we issue.
