Project Methods
The Open Project. Creation of the medical record When a patient joins Taw Hill Medical Practice, their personal details are entered onto a computer database at the practice and an electronic message is sent from the practice to the health authority registration department. Your previous general practice is contacted and your notes are "recalled". Once the registration department receives your old medical notes then they immediately forward them to us at the practice. Some notes arrive very quickly (after about 7-10 days) but others can take 2-3 months to arrive (~ this is usually due to slowness on the part of your previous practice to return the notes to the health authority). Before your old notes arrive, if you require medical services from us, we create an electronic medical record for you held on our secure clinical system. When we
receive your old notes, we sort through them and start to summarise
them. We try to extract important information ( past diagnosis ,
allergies / vaccinations etc) and prepare written and printed
information for scanning. All of this information is then attached to your electronic medical record and is available to the doctors and nurses who see you at the practice. When you request a copy of your medical record from us, we can export it electronically onto CD Rom disk. For a description of the technical aspects, please click here |
