You can call in to the practice between 10am-12pm and 4-6pm Monday - Friday to make an application to receive your medical records on disk.
You will NOT be provided with a copy of your medical records at this initial contact.
A digital photograph will be taken and a letter showing this photograph will be posted (to you) at your home address informing you by post that an application to receive a copy of your medical records is being processed.
Once
you receive this letter, you will need to bring it along together with
some other form of identification e.g. driving licence or passport or a
household utility bill showing your name and address in
order to collect your medical records.
We make no
apology for the rigorous nature of our checking procedures ~ the
accuracy
AND
confidentiality of your medical records is our prime concern. In the
event that the practice cannot satisfy itself beyond all reasonable
doubt that the medical record requested is that of the person collecting
it then we reserve the right not to release a copy of the medical record
until any element of doubt has been extinguished. Please note that we
operate a zero tolerance policy towards unacceptable
behaviour towards our staff.
