 Laser and Intense pulsed light treatment is only one method of treatment of acne rosacea which can vary greatly in severity/ type and distribution. For most patients, and many referring clinicians, being able to explain what laser or intense pulsed light treatment will be like can be difficult unless a treatment can be witnessed beforehand. The videos reproduced here have been taken and published with the with the kind permission of the patients who appear in them as an information resource for other rosacea sufferers. The development of this series of videos  followed suggestions from participants on the discussion forum at www.rosaceagroup.org

Intense Pulsed Light treatment of Acne Rosacea using The Lumenis One and TRIPLC Protocol.

Important note:- There are many different protocols for IPL treatment of Acne Rosacea and TRIPLC is only one such protocol. This protocol is not intended to replace clinical judgement (and the author accepts no responsibility for any damage caused by other practitioners adopting these treatment protocols in place of their own clinical judgement). Physicians must assess each patient's suitability based on their medical history, skin type, severity of rosacea and co-morbidity. The process involves three separate exposures of the patient's skin to Intense Pulsed Light - each time the treatment head is  rotated 90 degrees ( i.e. at right angles to the previous pass). The intention is to even out the delivery of Intense Pulsed Light Energy by off-setting each pass relative tot he one before. The change of filters between each pass is intended to deliver energy at varying levels in the skin (and so to target vessels of varying diameter and at varying depth). By using multiple passes, using moderate energy levels, we hope to successfully treat as many vessels as possible whilst minimising the risk of thermal damage to the skin.

This video shows a first vertical pass using the Lumenis One Machine using the 590nm filter at 24j/cm2 and triple pulse.

This video shows a second pass ( this second pass is done at 90 degrees to the first i.e. the treatment head is horizontal) using the Lumenis One Machine using the 615nm filter at 24j/cm2 and triple pulse.

This video shows a third vertical pass using the Lumenis One Machine using the 695nm filter at 24j/cm2 and triple pulse. Note, the 3rd pass with the 695 nm filter may be omitted over areas where hair loss would wish to be avoided.

Pulse Dye Laser for IPL Resistant Acne Rosacea using the Nlite Laser

This video shows the NLite 585nm pulse dye laser used to treat Intense Pulsed Light treatment resistant Acne Rosacea  (rosacea that does not respond well to Intense Pulsed Light treatment). The laser used in this way causes bruising of the blood vessels and this develops as discreet dark purple circular bruises as the treatment continues. The video below shows the appearance after treatment.

This video shows the effects of using the pulse dye laser in purpuric (bruising) mode. Any patient wishing to have treatment with the laser used in this mode must be prepared to have noticeable bruising for at least 7-10 days after treatment. Although this treatment can produce results where Intense Pulsed Light (IPL) alone has been unsuccessful, the evenness of the result is not generally as good as with IPL and IPL may be required to "feather" out the sharp cut off where the pulse dye laser has "punched" a hole in the redness which can other wise leave an uneven approach.

Before, During and After photos The first picture shows pre-treatment. This patient was not responding to conventional IPL. The second picture shows the purpura (purple bruising) caused by the laser immediately and for several days after treatment. The third photograph shows the appearance of the skin 19 days after treatment. Permission to reproduce these pictures kindly provided by the patient.

Dermetrics Blood Map picturesThese pictures were taken with the Siascope - a special camera applied to the skin and which affords selective mapping of blood vessels beneath the skin. This sequence shows 2 pictures taken after Pulse Dye Laser treatment. Notice how the pulse dye laser has a well demarcated treatment area and the 2d and 3d picture shows areas of blood vessels between the pulse dye laser exposures.

 

NewGemini 1064nm Nd:Yag Laser treatment New treatment to address vascular hypersensitivity and flushing for  a patient with acne rosacea.

Quicktime Video of TreatmentThis video shows treatment with the Gemini Nd Yag laser using the "painting mode". the video shows all the steps of the procedure from application of the treatment gel through the three passes - each pass consists of a horizontal and vertical pass. The third pass omits the beard area.

Dermetrics "Bood Map" PicturesThis sequence of pictures were taken with the Siascope - a special camera applied to the skin and which affords selective mapping of blood vessels beneath the skin. This sequence shows 5 pictures taken over 7 months during this time the patient had Gemini 1064nm treatments. The patient reports less flushing and the siascope pictures show steady progress which matches the experience of the patient.

 

KTP Laser for prominent facial blood vessels using the Iridex 532nm Diode Laser

This video shows treatment use of the K.T.P. 532nm diode laser to treat blood vessels on the face. In this example, the patient had severe rosacea treated with Intense Pulsed Light. Once the acne rosacea was under control, although the cheeks, forehead, nose and chin had improved dramatically, the reduction in the acne rosacea revealed several prominent blood vessels on the side of the nose. The K.T.P. laser shown in this video is used to draw over the blood vessels and the laser energy is selectively absorbed by the blood - the blood vessel clots instantly and often, the vessel just disappears instantly from view.
